ITC Care Coordinator

Murray Valley Aboriginal Cooperative (MVAC) was established in 1974 with a vision to provide health, housing, and childcare/education services to our community. As the largest service provider in Robinvale, we are committed to Child Safe Standards and to ensuring the safety and well-being of all children and young people in our care.

About this role

The Integrated Care Coordinator is responsible for coordinating holistic, person centred care for MVAC clients with chronic and complex health and psychosocial needs, working closely with the Primary Health Network (PHN), local hospitals, general practices, and community services to improve care pathways and health outcomes for Aboriginal people. The role provides navigation, care planning support, and system coordination, ensuring that services are culturally safe, timely, and responsive to community priorities.

Key Responsibilities

Essential:


  • Experience in Primary Healthcare and the ITC program, preferably in not-for-profit or community-focused settings.
  • Understanding of Child Safety Standards and child protection laws.
  • Strong communication and interpersonal skills to engage diverse stakeholders.
  • Experience working with Aboriginal and/or Torres Strait Islander people.
  • Competency in Microsoft Office Suite, email, data entry, and record management.
  • Current Working with Children Check (linked to MVAC).
  • Police Check (issued within the last 3 months).
  • Current unrestricted driver’s licence.
